Our priority is to assist families who might otherwise struggle to afford essential aids and equipment for children with a physical disability.
Physical disabilities impact a person’s physical capacity and/or mobility and can be congenital or acquired as a result of an accident. Aids and equipment considered essential for one person can differ greatly to the next. For this reason, all applications are considered and treated on an individual basis.
ELIGIBILITY:
Applications will only be considered where the child:
~ is under 18 years of age;
~ has a primary physical disability; and
~ resides in, and/or accesses medical services within, the Hunter region.
Applications that do not meet these criteria will not be considered.
REQUIRED DOCUMENTATION:
You will be required to provide:
~ At least one current quote (two quotes are preferred where reasonably obtainable) for the aids or equipment sought.
~ A supporting medical report (not more than 12 months old) from a General Practitioner, Specialist, or Therapist. This report must outline the child’s physical disability, explain why the equipment is needed, and describe the expected outcomes.
Photographs of the child trialling the equipment or aids are encouraged, where available, as they assist in the assessment process.
Application for Assistance
Please ensure
all questions are completed and all relevant documentation is attached.
Applications may be submitted by the child’s Parent/Carer or Therapist.
