Application for Assistance
Our priority is to assist families who might otherwise struggle to afford essential aids and equipment for children with a physical disability.
Please ensure all questions are completed and all relevant documentation is attached.
Eligibility:
In accordance with our Constitution, applications will only be considered where:
1. The child is under 18 years of age;
2. The child has a primary physical disability; and
3. The child resides in, and/or accesses medical services within, the Hunter region.
Applications that do not meet these criteria will not be considered.
Required Documentation:
You will be required to provide:
1. At least one current quote (two quotes are preferred where reasonably obtainable) for the aids or equipment sought;
2. A supporting medical report (not more than 12 months old) from a General Practitioner, Specialist, or Therapist. This report must outline the child’s physical disability, explain why the equipment is needed, and describe the expected outcomes.
Photographs of the child trialling the equipment or aids are encouraged, where available, as they assist in the assessment process.
Applications may be submitted by the child’s Parent / Carer or Therapist.
